Exemestane is a potent and highly selective third-generation, nonsteroidal antiaromatase drug. Originally developed as an antineoplastic drug for the treatment of breast cancer, aromatase inhibitors are also used by men to reduce blood estrogen concentrations and increase testosterone and gonadotropic hormone levels. By inhibiting aromatization, Exemestane prevents the conversion of testosterone to estrogen, a female hormone that, when present in excess, can lead to negative effects in the brain and hypothalamus. Exemestane cannot reduce already elevated estradiol levels, but it effectively prevents further estrogen formation.

At a dosage of 25 mg, Exemestane is a third-generation aromatase inhibitor. It creates a covalent interaction with aromatase during the first oxidation cycle, resulting in effective, selective, irreversible inactivation of the enzyme. This reduces estrogen synthesis in the body by 97%, making it more potent than anastrozole. Additionally, Exemestane is generally better tolerated by patients, with fewer side effects.
